How to make a complaint:

We encourage all families to speak directly to the Cooee Team Member who may be able to help them resolve the issue, although we acknowledge this might not always be suitable.

There are different options to make a complaint, or give us feedback:

 

You can submit your complaint via google form by clicking this link.  This form will be anonymous, unless you choose to enter your details.

You can email the Practice Manager on [email protected], or the Director on [email protected] with the details of your complaint.

You can speak to the Client Care team on reception, who can support you to make a complaint.

NDIS Participants can also access support to make their complaints via engaging a Disability Advocate.
